Failing Skull

I noticed this after I failed Pandemonium Hard at like 70% on my PS2. Once the life depleted sign went away, behind the Round Failed sign there was a skull which eventually faded away. Does anyone know why or when the Fail Skull comes up?

Yes, can confirm it only appears when failing pandemonium.

Does this just happen on the home version, do does it happen in the arcade too? If it does, do both ITG1 and ITG2 do it? Also, does this only happen on hard/expert or can it happen on medium/easy too, lol I have never failed it so I would not know

I haven't heard anything about it on arcade versions, and I'm going to assume it only happens on expert, since that's the 13-rated part.

On ITG 1, I know it happens on Expert if you fail.

Dunno about ITG 2 though.

Haha on ITG1 it happens on hard and expert, never failed medium or easy too find out. Don't know about ITG2...
