I have 2.

-Falling off the machine when i was doing Cartoon Heroes light. i failed soon after.

-After finishing LoM on Beginner, i was so tired i collapsed onto the machine. I hit it too hard, it banged into the wall, and the screen went black. I freaked out, i thought i broke it! Turns out I just reset it.

Well, I always feel a bit embarrassed when I play DDR around people that are worlds better than me, but I haven't had any really mortifying experiences playing DDR....the worst was when I fell of a PIU machine a year ago.

I did a faceplant on 5th mix screen in Philippines (Timezone Australia arcades) while playing HV AM... damn i tripped on my backpack i left the strap on the pad... all those girls laughing... but i still passed HV AM!

I was wearing platform shoes and I decided to take them off to play DDR. My socks were slippery and halfway through So Deep I slipped and fell

The other was when I failed Hyper Eurobeat standard because...I have no idea why...I sucked that day.

When I was in Wildwood NJ on vacation I played doubles but little did I know that stupid people dont wipe there feet when they play... so there was sand all over it and stupid me never looked at it. So I was playing doubles and didnt notice it and fell of 2 time's and looked like an idiot. But i still passed. yay me!

my most embarrasing ddr moment was when i was playing cartoon heroes standard and when i went to hit the upleft arrows i jumped to far and hit the corner of the machine and i slipped off and tore my knee up pretty bad.

i was playing abyss heavy and i fell right off during the galloping steps... but managed to limp back up and hitting the arrows with my fist while doing so, then got up and finished the song with a C... it was embarassing cause my friend i havent seen in a while was there and i was telling him "i got better dude"... :-P
